SCHOOL WIDE DISCIPLINE PLAN

  SCHOOL WIDE RULES

1. Attend school daily.
2. Be on time daily.
3. Follow directions the first time they are given.
4. Act respectfully toward people and property.
5. Be prepared to work.
6. Stay with supervising personnel.
7. Do all work assigned to the best of your ability.

   
CONSEQUENCES
   
Infraction #1 a) Verbal Warning.    
  #2 a) Warning with name on board.    
  #3 a) Time Out    
   
  Pre-K--Kindergarten 5 Minutes
  1st--2nd 10 Minutes
  3rd--5th 15 Minutes
   
   
b) Loss of Privileges
c) Student /Teacher Conference (Optional)
  Set up student contract (Optional)
   
  #4 a) Demerit 1 (See Parent Letter)    
    b) Call Parent/Guardian.
c) Record on Discipline Case Card/CMCD Case Log.
   
  #5 a) Demerit 2 (See Parent Letter)    
    b) Detention Center Assignment- During Ancillary
c) Three consecutive Detention Center assignments within one week will require a parent,
  student, teacher, and administrative designee conference.
   
    d) Record on Discipline Case Card/CMCD Case Log.    
  #6 a) Demerit 3 (See Parent Letter)    
    b) Teacher/Administrative/ Student/Parent conference is required. Administrator will
  determine appropriate consequences at the end of the conference.
c) A suspension letter will be issued for the fourth offense.
   
  #7


a) Parent or designee will monitor student in class for a minimum of four hours or the
  student will be suspended for 1--3 days.
b) Teacher Discipline Committee will determine appropriate consequences.
   


• Serious acts of misconduct such as those described in Levels III and IV of H.I.S.D. Student Code of Conduct must be investigated and documented. Send documentation to the administration for action.
• Students receiving U’s in conduct will be placed on a behavior contract that will be developed by the teacher, parent, and student. A copy will be submitted to the principal.
• After all suspensions, parent returns with student for re-entry into school.
• Extracurricular activities are designed for students that follow the School-Wide Discipline Plan and the School Dress Code. Extracurricular activities include: Field trips, school programs, clubs, class parties, field day, fall carnival, awards, and promotion ceremonies.


*NOTE: Parents and, students are to follow all School-Wide Rules.
Consistent failure to do so will result in excluding your child from any extracurricular activities.
Being tardy more than 10 times and more than 10 uniform violations are considered a discipline issue
which will result in your child’s conduct grade lowered and exclusion from extracurricular activities.
