                REGISTRATION  REQUIREMENTS

WE MUST HAVE Items 1, 2, 3 listed below IN ORDER TO PRE-ENROLL YOUR CHILD for Pre-K and Kindergarten. 

WE MUST HAVE Items 1, 2, 3, 4 listed below TO PRE-ENROLL FOR  grades 1 through 5. 

YOUR CHILD WILL NOT BE PRE-REGISTERED WITHOUT THESE DOCUMENTS.

1.      Current Lease (& parent ID) OR Current utility bill

2.      Original/copy  birth certificate

3.      Immunization records

          -    (4) DPT and (3) Polio (Last dose after age 4)

          -    (1) One Rubella (German Measles), Mumps (MMR)

          -    (2) Measles (may combine

               with MMR)

          -    (3) Hepatitis B

-    (1) Varicella (Chicken Pox)     (2 varicella for Kinder)

          -    HIB for 4 year olds (PK)

          -    (2) Hepatitis A for PK & KG

          -    (4) PCV7 for PK

4.    Last Report Card (For grades 1-5)

5.    Social Security card (If child has one)

NOTE:  For Pre-Kindergarten, the child must be 4 as of September 1.  The child must also qualify as either not speaking English, or qualify on low family income.

Su hijo(a) NO será pre-registrado sin estos documentos.
