REGISTRATION
INFORMATION
New Student Registration is scheduled
as follows:
Pre-K Registration ONLY: August 13
from 9:30-a.m. -2:00 p.m.
New Student Registration: August
14-24 from 9:30-12:30 each day
1. Proof of
residence (lease agreement of HL&P statement with
parent's name)
2. Birth Certificate for student
3. Shot record
-
DPT &
Polio series complete and last booster since 4th birthday
-
Rubella -
(German measles) may be combined Measles (Rubeola)
-
Mumps
-
Haemophilus
Influenza Type B (HIB)
-
Hepatitis B
(3 series) all students born on or after September 2, 1992
4. Social Security
card (optional)
5. Custody papers
signed by Judge
6. Last report card if
coming from another school
7. Copy of Texas
Driver's License
Full
day Pre-K requirements for attendance are:
1.
Students
who have limited English proficiency
2.
Students
who are homeless
3.
Students
who meet income eligibility

the front office.
