REGISTRATION INFORMATION

     1.  Proof of residence (lease agreement of HL&P statement with parent's name).

2.  Birth Certificate for student

3.  Shot record

•        DPT & Polio series complete and last booster since 4th birthday
•        Rubella - (German measles) may be combined Measles (Rubeola)
•        Mumps - 
•        Haemophilus Influenza Type B (HIB)
•        Hepatitis B (3 series) all students born on or after September 2, 1992

      4.  Social Security card (optional)

      5.  Custody papers signed by Judge

      6.  Last report card if coming from another school

      7.  Copy of Texas Driver's License
