Requirements for Enrollment 2008-2009

All Grade Levels

8:00 a.m. - 12:00 p.m.

Requirements for Enrollment

  • Certified Copy of Birth Certificate (original, we will make a copy)
  • Immunization (original, we will make a copy) child must be up to date
  • Two polio + booster after the age of 4
  • Three DPT + booster after the age of 4
  • Two MMR after the age of one (at least one month apart)
  • Three Hepatitis B
  • Varicella or history of illness
  • Child’s Social Security Card (original, we will make a copy) HISD will assign another 9 digit number if we are not provided your original social security card.
  • Most recent report card or any testing results indicating grade level or vanguard placement.
  • Parents/Guardian Driver Licenses or Passport (we will make a copy)

  • Three proofs of Residency (no exceptions, we will make copies)

1. Electric bill showing residential service with name and address

or confirmation letter of application on utility company letterhead (no printouts from your computer)

2. Gas or Water bill showing residential service with name and address or confirmation letter of application on utility company letterhead (no printouts from your computer)

3. Harris County Appraisal District statement or receipt showing Homestead Exemption (If you are purchasing a home I will accept the signed closing statement or contract with dates for the actual closing of your home).

4. If you are currently leasing a home/apartment the lease must list all occupants and cancelled check (back and front) or receipt for current month’s rent.

NO INCOMPLETE ENROLLMENT DOCUMENTS ACCEPTED

Boundaries for West U residency  Kirby Drive on the East, Bellaire/Holcombe Drive on the South, the RR tracks on the West, and Westpark jogging at Edloe to Southwest Freeway on the North.

Three Proofs of Residency

  1. Electric bill showing residential service with name and address
  2. Gas/Water bill showing residential service with name and address

  3. Harris County Appraisal District bill or receipt showing or Homestead Exemption

or

  1. Current lease showing occupants with cancelled check for most                       current month’s rent

IF HCAD Homestead documents have not been received, we will accept the signed Closing statement

No incomplete enrollments accepted during early registration.
