Students coming from another H.I.S.D/ school will need:

       1.  Last year’s final report card
       2.  Proof of address (utility bill, rent receipt, etc.)

Students coming from another school district will need:

       1.  Child’s original birth certificate
       2.  Record of child’s immunization signed by a doctor
       3.  Proof of address (utility bill, rent receipt, etc.)
       4.  Last year’s final report card and school’s address
       5.  Social Security number (optional)
