• The person enrolling a student must be the parent or legal guardian.
• Student must reside within the school's attendance zone.
• Social Security Card (if available).
• Proof of Residence (We can take utility bills or a lease agreement; it must have parent's name and address).
• Appropriate immunization records.

• WE DO NOT TAKE RENT RECEIPTS.

It is important that enrollment cards be filled.  Include at least two numbers so that we can contact you immediately in case of an emergency. If you have a pager or cellular telephone, please provide us with the number in case of emergency.  Your doctor's number should also be included.  If your telephone numbers or address changes during the year, send the changes to the office as soon as they occur.

In case of early dismissal or an emergency, students will be released only to the person or persons who are listed on the enrollment card.
 

• A copy of your permanent record
• A copy of your latest report card
• A copy of all TASK scores
• A copy of all Stanford 9 scores
• A sample of student work (an essay, project, or report from current year)
  
  Students new to HISD must also submit:
   
• Proof of address
• Immunization Record
